Fault Code 1151 on 98 Contour SE

Ok, have been battling an 0401 fault code for a while. Last night, got down to it and cleaned the EGR passages (which were loaded with crud), I have replaced the EGR Valve, EGR Sensor, Bank 1 o2 sensor, pcv valve, and cleaned the throttle body, and after about 10 miles of driving today, boom, CEL back on. So I ran over to AutoZone and am getting a P1151 fault code. I can't get through inspection with the CEL on.

I have a 98 Contour SE V6 DOHC. Just turned over 200,000 miles yesterday.

The print out from AutoZone says:

Definition:
Lack of o2 switches: Bank 2 Sensor 1

Explanation:
ECM could not adjust fuel trim because of a lean or rich condition

Probable Causes:
1. Fuel Pressure high or low
2. MAF Sensor dirty/defective
3. Vacuum leak on engine
4. leaking fuel injector or fuel pressure regulator

HELP!!!!!! Has anyone come across this before and what is the remedy???? I am at my wits end!
 
Whew! That will be easy compared to everything else I have done. Should I go ahead and replace the 2 by the cats as well, just because?
 
Replaced the O2 sensor, drove about 20 miles, and CEG back on. What should I try now? I am not sure what the fault code is, yet, as it is now getting late.

Any thoughts about what it could be?
 
Need the exact code(s) or we are shooting in the dark (pardon the pun) .:D
 
Thanks for the quick reply Tony. I am glad you are looking at my thread. I am hoping you can help. I will swing by Autozone on my lunch hr tomorrow and have the code read and repost after lunch.
 
Actron makes a cheap code scanner, lets you read/clear the stored codes. You can also remove a fuse to clear codes (maybe Tony knows which?) The code will stay a while otherwise.

I think the Actron 9125 (??) is about $70. It only reads and erases the codes. If you want to get something good, get the 9180. It is the latest and greatest for about $200. Read, live data, freeze frames and erase. Internet upgradeable. I am thinking of getting the 9180 if I can return the 9145 I bought a month ago (still in its case).

Don't erase the code (s) yet. You want Autozone to read it so that you can post.
