plenty of auto parts stores should carry a headlight restoration kit.
No need to remove the covers or light (that is if the fog is on the outside) just leave the lights in the car and clean them up with one of the available kits.

By cover you mean the headlight. All it takes is some wet sanding and some polishing. Try and find the 3M kit that uses a drill.

For mine, which were yellow but now so yellow that you couldn't see through them I did....
1) Used Automotive sandpaper from Wal-Mart. I started with 1500grit then 2000 grit and finished with 2500 grit. Some people start with 1000grit if there lights are really bad. I would say that I initially spent 10 minutes per light for piece of sandpaper.
***Note: Sand in horizontal and vertical motion, NOT circular.
You should see that the yellow is gone but they are really hazy.
2) I used Mothers Mag/Aluminum wheel polish then and liberally applied and rubbed it into each headlight.
3) I finished with Meguire's new PlastiX polish and my lights are much much clearer however there is still the crow's feet that won't come out.
4) About every 3 months I still/will use the PlastiX to clean up the lights from the yellowing.

The fog may look like its on the inside, but its not. That discoloration is perfectly normal for plastic headlights if they arent buffed and waxed along with the paint. Clean them with some non-ammonia windex and then start wet sanding like we're saying. You have to sand off that top layer that discolored and then polish out the sanding marks with plastic polish.
This is the 3M kit I was talking about and its by far my favorite because you get the nice foam applicator.

If they are truely foggy on the inside, then you have damaged seals and moisture is getting in.

And you could always pull a bulb and see if it dries out if thats the case.

Still needs to be buffed with somthing to remove scratches. Think I should try tooth paste or baking soda?
Mine is not a show car or fancy at all so test this on your car if you decide to try.

You should try a plastic polish. In a pinch any polishing compound but the plastic polish will help seal the lens so it doesnt just glaze over in a few days.

Spray some clear coat on them now. It will fill the scratches and make them very clear. Just make sure you have sanded through the layer that turns yellow.
Before- sanded with 2K grit until the yellow layer is gone.

The Blue Magic stuff does work really well, far better than the PlastX . Gets the fine scratches out much better . I did the 1000 - 1500 - 2000 wetsanding thing and then finished with Plastx originally. Looked good but not great .
Touched up my lights a few months later with BM only and they look like new almost, noticably better than the PlastX results.
The PlastX is susposed to have a sealent in it though, so next time I'll probably do BM and then PlastX over top of that.

Be sure to hit them with a good wax as well. Gives them some extra uv protection.
